Move the remaining bits of &lt;sys/diskslice.h&gt; to &lt;i386/include/bootinfo.h&gt;

Move i386/pc98 specific bits from &lt;sys/reboot.h&gt; to
&lt;i386/include/bootinfo.h&gt; as well.

Adjust includes in sys/boot accordingly.
