These includes were for __FBSD_RCSID() macro. They weren't formatted
like the rest of the tree so weren't trimmed automatically when that
script was run. Trim them now.

The SPDX folks have obsoleted the BSD-2-Clause-FreeBSD identifier. Catch

Name each ehci driver uniquely.
This remove the warning printed at each arm boot :
module_register: cannot register simplebus/ehci from kernel; already loaded from kernel
A similar fix was done in r333074 but imx_ehci wasn't renamed and generic_ehci wasn't
present at that time.

Mainly focus on files that use BSD 2-Clause license, however the tool I
was using misidentified many licenses so this was mostly a manual - error
prone - task.
The Software Package Data Exchange (SPDX) group provides a specification
to make it easier for automated tools to detect and summarize well known
opensource licenses. We are gradually adopting the specification, noting
that the tags are considered only advisory and do not, in any way,
superceed or replace the license texts.

It turns out that this is more than a power optization. The OTG port
won't work on boards that have this property unless this setting is honored.
Also ensure that the usb phy device attaches before ehci.

This enables the PHY circuitry for UTMI+ level 2 and 3, and sets the
flag to tell the ehci code that the root hub has a transaction translator
in it. For imx6 we can use the standard ehci_get_port_speed_portsc()
function to find out what speed device is connected to the port.

define a few imx_ccm_foo() functions that are implemented by the imx51 or
imx6 ccm code. Of course, the imx6 ccm code is still more a wish than
reality, so for now its implementations just return hard-coded numbers.

Simplebus used to only instantiate its children whose status="okay"
but that was improper behavior, fixed in r261352. Now that it doesn't
check anymore and probes all its children; the children all have to
do the check because really only the children know how to properly
interpret their status property strings.
Right now all existing drivers only understand "okay" versus something-
that's-not-okay, so they all use the new ofw_bus_status_okay() helper.

than one unit with four busses attached to it. This allows us to use
existing fdt data which describes separate devices with separate resources.
It also allows any combination of the units to be en/disabled in the
board dts files.
Adjust our dts code to match what's used by linux and u-boot now that
we're structured to do so.
Document lots of interesting stuff learned whiling doing this with a big
comment block in the driver, so I don't have to re-learn it for the next
round of changes.
